Corrections, translation and rejection
Do not use whiteout. Cross out an error and initial it. Staff cross out unused blanks when commissioning to prevent later additions.
The city does not translate documents or the oath. If a person cannot understand the oath, they must supply an English-speaking translator for their language. The translator signs an additional document with its listed fee.
The commissioner confirms the oath process, not the truth of the content. If another organization rejects the commissioned form, the city does not refund or recommission a new form for free.
